Robert Beukema

DNP, PMHNP-BC

Ten years at the bedside before I ever wrote a prescription.

I hold a Doctor of Nursing Practice and I'm board-certified as a psychiatric nurse practitioner, not a physician.

I diagnose and prescribe by video, stimulants included.

Why I Do This Work

I prescribe propranolol for performance anxiety because it blunts adrenaline's effect on the body. It turns down the physical symptoms: racing heart, shaky voice, sweating through situations you're prepared for. Your body stops getting the final say.

I didn't recognize my ADHD until my 30s. It didn't match what I expected: no "racing thoughts," just unfinished projects, memory gaps from constant distraction, difficulty sitting down despite knowing exactly what needed doing.

I tried Adderall. It helped with focus but made me irritable with the people I care about. For me, the trade-off pointed toward non-stimulants. For you, the math might be different.

Not pushing medication regardless of cost. Not withholding it because of stigma.

Just an honest look at whether a specific tool removes your specific barrier.

From Bedside to Prescriber

I learned psychiatry on a hospital's inpatient unit. I stayed for a decade.

As a psychiatric nurse, I watched thousands of medication decisions play out in real time, from the bedside where you see what actually happens. Which side effects become deal-breakers. What happens when treatment gets too complicated. Why most medication failures aren't about picking the wrong drug.

Crisis psychiatry added something else: precision about who needs intensive intervention and who needs good outpatient care. You're probably in the second category.

Nursing is still my foundation, and that path is the reason I prescribe the way I do.

Data, Not Guesswork

Decisions made from data stay steady when feelings swing.

I measure where you start and again at every check-in. When a rough week makes everything feel worse but the scores hold steady, that's worth a closer look, not a reason to tear up the plan.

The week swings. The scores hold. I look closer before I change anything.

That's how Solana Psychiatry manages medication, and it's the part of this work I trust most.
