    Tell us which days and times suit you, using the form below. We'll confirm within 1-2 business days.

You pay after your visit, not when you book

Your time is set once you book it on the calendar or we confirm your request. Need to cancel your evaluation? Give us at least 24 hours. Later cancellations cost $25, and a no-call no-show is charged the full appointment fee. We take credit cards, HSA/FSA cards, and electronic payments.

Add an email address, a phone number, or both so we can confirm your time.

Simple privacy promise: We use your information only to schedule and provide your care. We don't use it for marketing, and we don't sell it. It's sent over an encrypted connection and isn't stored on this website. More in our privacy policy.

Never been diagnosed? Your provider can diagnose you at this visit, no psychological testing first. Already on medication? You start here too. You get a treatment plan, and medication if appropriate. If we can't help, you don't pay for the visit.

Initial evaluation

  1. Get a time. Book it on the online calendar, or send a request and we'll confirm or suggest another time within 1-2 business days.
  2. Complete your intake forms. After your time is confirmed, we'll send you a link. The forms cover consent forms and practice agreements, your basic details and contact information, current medications and pharmacy, and your psychiatric and medical history.
  3. Your evaluation. About an hour by video, ending with a treatment plan.
